This is about a 30-page legal document that includes background information, including a Motor Vehicle Report, Background Investigation and Investigation of previous employers. Once your DRIVER QUESTIONNAIRES are completed, we will upload this document to your mobile app. . We recommend keeping a copy at the business address on the application. The cost associated with having this Driver Qualification File created is $199.00 per driver. This document is valid for 12 months, the renewal cost is $159. The Driver Files will be required to pass your Department of Transportation Safety Audit.Note: Company Owners are required to have a complete DQF as well.
